Monthly Membership
Pre-tax HSA can be used to cover all or part of your membership.
$125 per individual.
LARGE FAMILY DISCOUNT- family membership caps at $300 for parent(s) and all dependent children.
$75 one-time enrollment fee. No enrollment fee for dependent children of members.
